Application for and issuance of license
(1)
A sworn application for a license as a motor vehicle service agreement company shall be made to and filed with the office on forms as prescribed by the commission and furnished by the office.(2)
In addition to information relative to its qualifications as called for under s. 634.041, the application shall show:The location of the applicant’s home office.The name and residence address of each director, officer, and 10-percent or greater stockholder of the applicant.Other pertinent information as required by the commission or office.(a)

The application when filed shall be accompanied by:If a corporation or partnership, a copy of the applicant’s articles of incorporation or partnership document, if any, certified by the public official having custody of the original and a copy of its bylaws certified by its secretary.A copy of the most recent financial statement of the applicant, verified under the oath of at least two of its principal officers.The license fee as required under s. 634.071.(a)

Upon completion of the application for license, the office shall examine the same and make such further investigation of the applicant as it deems advisable. If it finds that the applicant is qualified therefor under this part, it shall issue to the applicant a license as a motor vehicle service agreement company. If the office does not so find, it shall refuse to issue the license.
